Senior Transitions provides a variety of services associated with managing the care of older and disabled adults. These services are designed to promote better health and safety for our clients. At Senior Transitions, we believe in the old saying, “an ounce of prevention is worth a pound of cure.” Our elder care managers are experienced nurses who can accompany clients on doctor and hospital visits, and consult with physicians and other care providers. Even though your loved one may be residing in an assisted living facility or a nursing home, you still have the right to monitor the care that they are receiving. Senior Transitions’ elder care managers are available to go into the facility, check on your loved ones and monitor their care. Our elder care managers are also available to go into our clients’ homes and conduct assessments, write care plans, set up medications and monitor medications for side-effects.
